What a Donor sees in their Donor Portal when opting in or out of text messaging.

This page shows the actual Communication Preferences screen from the Benemens Donor Portal, reproduced here because that screen requires a Donor to sign in with a one-time email code first — there's no way to view it without an authenticated session. Copy, layout, and default (unchecked) toggle states below match the live screen exactly, so carrier reviewers can verify the web-form opt-in flow described in this campaign's registration without needing donor credentials. This includes the mobile number field itself: a Donor must enter and verify a number by SMS code before either text toggle can be turned on, so every number opted in through this screen is confirmed to belong to the person giving consent.
